Community Learning Disability Nurse, West London NHS Trust

I work within a community team with a range of other disciplines, I work with my own case load and support individuals with a wide range of needs. I support coordination of healthcare, holistic care that encompasses emotional wellbeing. My role is varied, I offer support in an individual’s own home, clinic setting or hospital. I work to complete nursing assessments and care plans which lay out person-centred health outcomes to improve or maintain a person’s physical and mental health. I attend joint visits and assessments as required and work closely with our social care colleagues.

I would like to continue to further my post qualification development; the learning disability field is generally a smaller field and keeping up to date with my wider professional circles is vital to my growth and the growth of the wider field. I do believe together we are better and it is something I like to draw on in my practice. As a learning disability nurse, and a person with their own lived experience, I want to always remain a person centred creative clinician. I would like the programme to help me widen those ways of thinking, which will support me to share those skills with others. We lead by example and I would like to continue my professional development with as many skills as possible. This enhanced skill set will help me to provide enhanced care to those I support, their family and carers.
