Specialist Practitioner – Community Learning Disability Team, Cheshire and Wirral Partnership NHS Foundation Trust
I have recently transitioned into a community learning disability post, my prior experience has been within NHS general acute organisations as a learning disability liaison nurse, safeguarding and mental capacity practitioner. My present role is leading a 12-month project commissioned by the local CCG, the project aim and focus is around local authority learning disability provision, such as short breaks and day services. The role will support with the management of individuals with complex health needs across these setting to ensure positive outcomes and good service delivery within the locality. In addition to the project, I will be working alongside the wider MDT of the CLDT in preparation for my integration to a practitioner within the team following the completion of the project.
My hopes and ambitions for the future gravitate to a strategic focus within clinical provider services and emphasis on quality improvement. Using my previous experience within secondary services, I am passionate about leading and supporting the growth of the physical health pathway within community learning disability services, by ensuring national and local agendas relating to improving the health needs of people with learning disabilities are embedded into services.
My experience has predominantly been in leading change independently, I hope to achieve development and skills around leading a team/service to change and success, in addition developing my current skills around service development within a mental health and learning disability NHS foundation trust.
The development of my skillset will support the primary, secondary and specialist services to embed further acknowledgements to the physical health pathway within provider services. I hope the programme will support my vision for the next five years of my career within LD provider services.
