Community Learning Disability Nurse, Wiltshire Health and Care- CTPLD, Marlborough

My role as a band 5 community nurse involves assessing and coordinating the health and wellbeing of individuals under the Continuing Healthcare framework. A key part of my role is to engage with individuals, their families, and carers in order to coordinate care as well as CHC and healthcare reviews. I work collaborate closely with professionals and other services (including as part of a MDT) to assess and facilitate the care needs of individuals on my caseload to ensure delivery of the most appropriate care and support in a holistic and person-centred approach. As a community nurse, I am responsible for my own caseload, and I am empowered to make autonomous decisions which enables me to work flexibly to meet the needs of the service users.

As a qualified learning disability nurse, I am keen to capture opportunities that will support me to develop my leadership skills. I believe the FoNS programme will provide me the opportunity to develop these skills. This programme will help me:

  • Develop my ability to contribute to the development of Trust wide policies and procedures
  • To have the courage to speak out and challenge effectively and professionally in group meetings.
  • Gain confidence and experience that I hope will improve my practice and ability to support my team and people with learning disabilities and their families/carer in my current role
  • Prepare for the future and possible leadership roles.
