Jan Dzieza

I work for the North Hampshire Community Learning Disability Service as a Community Learning Disability Nurse and manage caseload of persons with complex health needs – physical health needs, mental health, behaviours which challenge, forensic background. In my work I widely collaborate with other professionals within my team such as psychiatrists, psychologists, occupational therapists, speech and language therapists and physiotherapists.

Very often my work requires cooperation with other services involved in supporting of persons allocated to my caseload. I happily take role of lead during care programme approach process and engage with care providers, GP surgeries and specialised services.

I am extremely excited about this programme and I think it will deliver great opportunities to develop my leadership skills. I strongly believe that high individualisation, which is the core of this programme, will also help me to formulate my aspirations within my organisation.
