Community Transition Nurse, Barnet, Enfield & Haringey NHS Foundation Trust
I work in an integrated learning disability team of nurses, doctors, social workers, OTs, psychologists and many other health and social care professionals. Within my community nursing team, my role is focused on transition. My role as a community transition nurse is to support those with complex physical needs as well as learning disabilities (and their families) to transition from children’s services to adult services. My role involves completing a comprehensive transition assessment that covers all aspects of the service user’s life and allows us to identify any unmet needs for input or further assessment. The assessment also provides a detailed summary of all the service user’s needs that will be useful to new professionals. I also closely liaise with the school, GP, the family as well as any other professional or service provider in the services user’s life to ensure a continuation of care. I ensure that I have regular contact with the service user and their family in addition to closely associated professionals by doing face to face visits.
This programme will be the beginning of my leadership journey. It will give me an introduction to exploring where and how I can develop in my career. I hope this programme will increase my confidence in my existing skills surrounding leadership and how to apply them but also offer some encouragement to aim for further learning. This programme will create opportunities and space for reflection which is not always prioritised outside of a busy work schedule. I believe I will really benefit from this opportunity as I will be able to acknowledge my strengths and weaknesses and any area I can develop or seek support in. I also hope to network with other RNLDs and hopefully establish a network across trusts to exchange learning.
