My role as a band 5 community nurse involves facilitating and coordinating the health and wellbeing of individuals families and carers. a dominant part of my role is engaging with individuals in order to coordinate healthcare reviews/care plans and working inter-professionally and as part of the wider MDT. We coordinate with others in similar projects to best enhance services and much more!

This programme will be brilliant in supporting me to enhance my leadership for the benefit of the team and the people we work with. The support provided by FoNS and the time to work on a project will be essential to my learning and development. I am a naturally reflective individual but am looking forward to benefitting from further guidance around realising my leadership aspirations.

I would like to develop either a sexual health, easy read documentation or annual health check project. These have been identified as areas requiring improvement within our trust. I’d like to set up a focus group and work collaboratively to promote inclusion.
