My present role is band 5 Children’s Community Learning Disability Nurse. My role entails collaborative working within a multidisciplinary team of clinical psychologists, consultant child and adolescent learning disability psychiatrist, specialist community learning disability nurses and systemic practitioners.

Our team provides assessment, evidence-based interventions and care planning across home, education and social care including initial health needs assessment for each child, referrals for appropriate assessment and examination and signposting families to services and resources.

In my experience, the heath needs of individuals with learning disabilities are becoming more complex. This requires collaborative working across health and social care services to meet these needs and to provide adequate care. As a team, we have the opportunity to work collaboratively across tier one services using our specialist knowledge of health inequalities and health outcomes for clients with learning disabilities.
