Community Learning Disability Nurse, Chapel en-le Frith, Derbyshire
I work alongside other nurses and professionals to assess the care needs and risks of referred individuals to ensure the most appropriate care is delivered in a person centred, holistic way. Some of my main responsibilities include, acting as the named nurse within the MDT, developing collaborative, needs based care plans through person centred assessment, planning, implementation and evaluation. Once developed, therapeutic clinical based interventions, which meet the complex needs of the individual, can be delivered.
I feel the fellowship would benefit me, my team and those I work with in a number of ways. I hope to develop skills and potentially add value to my current role through the form of:
- Clinical Leadership
- Patient care
- Professional practice
- Practice development
As I am a dual qualified practitioner, I also feel the fellowship could support both my registrations as a social worker and RNLD. Through the programme I would like to develop further within my career building on:
- My ability to support others with interpersonal skills in managing conflict and resolution.
- My ability to contribute to the development of Trust wide policies and procedures.
- My ability to challenge practice which may be out-dated or no longer appropriate by developing effective change management strategies to continuously improve practice and service provision.
- Support me to have the courage to speak out, challenge and act when things feel wrong.
