Clinical Team Lead, Early Intervention Community Team, Birmingham
I’m a registered nurse and currently a clinical team lead for the Early Intervention Community Team West based in Ladywood Birmingham. I manage a multidisciplinary team consisting of nurses, physiotherapists, occupational therapists therapy support workers, rehabilitation assistants, social workers, administrators, advanced clinical practitioners and clinical case managers. We are a community based team and our aim is to deliver effective intermediate care in a patient’s home (Home First ethos) and help them to regain their independence. This is in line with the vision of the early intervention programme for older people to be as happy and healthy as possible, living self-sufficient, independent lives, able to have choice and control over what they do and what happens to them.
I have joined the fellowship programme hoping to gain knowledge and develop new skills to facilitate ongoing staff engagement, change management and improvement. I hope to put these into practice to further support and develop my team ensuring they feel safe and heard in their area of practice. This will in turn facilitate team cohesion and better work cultures/environment.
I hope achieving this will reflect in the standard and quality of care delivered to the people we care for, ensuring they are safe and happy.
