In my role within my community learning disability team, I have a caseload of individuals with a learning disability who have been through a referral process and are deemed to have a physical health need which they need supporting with. i offer support such as screening, sign-posting to primary services and/or supporting to access primary services, and supporting other services to support that individual. I also offer physical techniques under best interest in conjunction with the mental capacity act to enable an individual to have bloods taken.

As part of a multi-disciplinary team, we can always get help and advice from other disciplines to offer a holistic and varied way of working with the individual.

I am hoping this programme will enhance my skills and confidence in leadership and future professional development to ultimately enhance the standards of care we offer.
