I am a senior community learning disability nurse working in a crisis and intensive support team. I undertake the role of a care co-ordinator, crisis worker offering interventions such as positive behaviour support plans, advice surrounding severe challenging behaviour, mental health needs and forensic backgrounds and discharges. These interventions are delivered in a community setting that supports people, preventing further hospital admissions and ensuring service users are in their identified preferred and home area (as identified as apart of transforming care). Green light working with relevant professionals and stake holders ensures that any admissions needed are supported throughout and that service users are integrated back into the community when safe and supported.

I would like to gain further knowledge and skills to continue to support my team and the other professionals I work with. I feel  this will be especially beneficial during a transitional period of our service and the re-shaping of the team. I think the programme will aid me to become a leader and ensure my practice is maintained to a high standard. I am looking forward to using the skills learnt to “think outside the box” so we are able to provide the best practice for our complex service users.
