Community Learning Disability Nurse, Norfolk Community Health and Care NHS Trust

I am working as part of a community-based MDT team; there are five Community Learning Disabilities Teams (CLDT) in Norfolk with health and social care professionals to support adults with a learning disability. The team provides specialist health and wellbeing input for adults with a learning disability whose needs cannot be met by mainstream services. Dependent on need, the team may offer direct intervention, indirect work with carers and families or joint working with other professionals and signposting or support to access mainstream health services.

We assess an individual’s needs and risk factors to produce multidisciplinary care plans to reduce risk while promoting independence, dignity and quality of life.

I am looking forward to:

  • Creating opportunities for the team and me to benefit people using the services, the Crating caring Cultures model and resources will enhance the services provided to deliver person centred care
  • Enhancing my skills to be able to lead the team and support them including mentorship skills for the benefit of the people using the services, families, carers, residents, relatives, and friends
  • Reflecting on all aspects of care given pre and post treatment
  • Being confidently able to understand the legislation involved at a local, regional and national level to deliver the caring culture safely and effectively
  • Being able to enhance and be confident in personal developing planning with the support from colleagues
  • Being able to confidently advocate and developing leadership skills when working with the interdisciplinary team and taking on board other professionals’ judgements
