Community Learning Disability Nurse, Kent Community Health NHS Trust

I am a Community Learning Disability Nurse, supporting people with learning disabilities in the community to meet their health needs. As learning disabilities nurses, we carry out health screening assessment for patients, risk assessment and refer patients to the appropriate team if other health needs are identified. This can include referrals to and occupational therapist, physiotherapist, speech and language therapist and epilepsy specialist nursing services.

As an LD nurse, part of my job is supporting patients in attending appointments, providing reasonable adjustments for patients and communicating with other professionals involved in the care of the patient so that they get the right treatment and that their health needs are met.

Part of my job role is providing training to clients, their family, other professionals and services.

I am hoping to become more competent in my job. I think the fellowship and supporting programme will benefit me so I am able to work more efficiently with patients, their families and others.
