Community Learning Disability Nurse, Livewell Southwest, Plymouth

I work in a community LD team alongside a number of other professions including SLT, psychiatrist, occupational therapists and support workers. We support individuals and their support networks with physical and mental health needs, sometimes in conjunction with adult services and CCG. Once referred to nursing, we may complete screenings or assessments to highlight areas that need addressing. We deliver health education, facilitate health appointments, or complete a referral to another service.

I hope that the programme will provide me with methods of more effective communication and leadership, which I can therefore use to better my nursing skills and provide a service that is more beneficial to service users and their support networks.

I feel passionately about ensuring those with learning disabilities are provided with excellent quality care. At times when people with learning disabilities can be overlooked, leadership is required to ensure the rights of these individuals are upheld.

I am hoping to establish a professional network within my team and in the local area. As I am new to the team, this will enable me to deliver a better level of care by obtaining differing perspectives.
