Senior Community Nurse, Doncaster Crisis and Intensive Support Team

I currently work in the Crisis and Intensive Support Team within the Learning Disability Team at Doncaster. I am a senior community nurse and responsible for the care co-ordination of my patients. My team supports individuals with severe challenging behaviours, mental health needs and forensic backgrounds in need of intensive support or in times of crisis. This is a community based service that supports people with where they live, preventing hospital admissions under the mental health act 1983 and avoiding patients being placed out of area where possible. In some cases, a hospital admission under the mental health act may be required and at such times, we support our patients throughout this process and look at integrating them back into the community as soon as possible, when safe to do so, to prevent long stays in hospital.

I would like to gain further skills in continuing to support and inspire the professionals I work with to be the best they can be. I also want to gain and share the knowledge of how we can improve our practice by innovating new ideas and positively transforming the way we work where there is room for improvement. Finally, I would also like to improve my practice where possible to ensure I am providing high quality, evidence based care to meet my patients’ needs.
